NHS App
If you’re a patient, you can use the NHS App on your phone or tablet. It’s an easy and safe way to access a range of NHS services. You can get advice about your health, order more medicine if you need it, keep track of appointments at the hospital, look at your health records, and more. If you already use Patient Access, you can still use it, but you can also use the NHS App if you want.
More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

Proxy Online Access
Patients can choose someone like a carer, relative, or adult they trust use their online services. The patient can decide which services they want this person to use. It’s important for patients not to share their own login details with anyone else. Instead, the person they choose will get their own login details to access the online services for them.
Both the chosen person and patient need to fill out a Proxy Online Access Registration Form and provide x2 forms of ID at Reception (1 photographic and 1 address, or 2 address ID). If the practice decides not to let them have access, the practice will contact the chosen person and advise them of the reasons why this decision has been reached.
Parents can ask to see their child’s health records online until the child turns 11. After that, it stops automatically. If parents want to continue online access, the child may have to give consent. The GP might discuss with the parent/guardian and grant access if it’s in the best interests of the child.
If access is again granted to the parent/guardian, it will stop automatically once the child turns 16.
